Leaving the assembly point as a vague description ("the parking lot")

Name a specific landmark and orient it relative to the building — e.g., "northeast corner of the visitor parking lot, next to the flagpole, at least 100 ft from the building."

No secondary assembly point if the primary is unreachable

Designate a backup that is on the opposite side of the building. Wind direction matters for any facility handling chemicals or with combustion equipment.

Missing accommodations for non-ambulatory employees or visitors
