How to Complete a Construction JHA

Six steps from blank form to a signed, posted JHA.

1. Break the Job Into Steps

Identify each major task in the work sequence — site setup, excavation, framing, electrical, roofing — and list the steps in order.

2. Identify the Hazards

For each step, consider falls, struck-by hazards, caught-in/between hazards, electrical hazards, chemicals, equipment, and environmental conditions.

3. Choose Hazard Controls

Use the hierarchy of controls where appropriate: elimination, substitution, engineering controls, administrative controls, then PPE.

4. Identify Required PPE

Document the task-specific PPE each step requires — fall protection, gloves, eye protection, respirators, hearing protection.

5. Review the JHA With the Crew

Walk through hazards and controls with everyone on the task before starting work, and again whenever conditions or tasks change.

Both terms describe the same pre-task hazard-identification process: break the job into steps, identify the hazards at each step, and document the controls. Whether your client asks for a construction JHA template or a construction JSA template, this online form works for both — some organizations also call it a Task Hazard Analysis (THA) or Activity Hazard Analysis (AHA).

Construction Hazards to Include in Your JHA

Work through this list for every task — OSHA's Fatal Four (falls, struck-by, electrocution, caught-in/between) cause the majority of construction deaths, but they're far from the only hazards worth documenting.

OSHA Construction JHA Guidance

OSHA does not publish a mandatory JHA form, but 29 CFR 1926.20(b) requires construction employers to initiate and maintain accident-prevention programs — which means evaluating workplace hazards before work begins. OSHA's own job hazard analysis guidance (OSHA Publication 3071) recommends exactly the process this template documents: break the job into steps, identify the hazards, and record the controls.

This construction JHA template is built on those OSHA hazard-analysis principles and cites the relevant standards (fall protection 1926.501, excavation 1926.652, scaffolds 1926.451, steel erection 1926.760, silica 1926.1153) in each example. It is a starting point for your own site-specific analysis — not an OSHA-approved or certified document, since OSHA does not approve or certify templates.

When Should a Construction JHA Be Updated?

A JHA should be reviewed when:

The task changes
New equipment is introduced
Site conditions change
A new hazard is discovered
An incident or near miss occurs
A new crew begins the task
Work moves to a different area

Don't treat your JHA as a one-time document.
